Academic Accommodations
Academic accommodations are modifications, adjustments, services, or aids designed to eliminate disability-related barriers in the environment and curriculum, so students have an equal opportunity for success.
Registering with FSU’s Student Accessibility Office for Academic Accommodations:
Both incoming and current students at Frostburg State University may request accommodations through the Student Accessibility Services Office using the following steps:
- Complete the Accessibility Accommodation Request Form and provide medical documentation or a psychological (psychoeducational) evaluation. Your medical provider can upload all documentation using the Provider Portal link.
- Any information/documentation that is readily available can be included with the form.
- Please review our documentation guidelines to learn what information is most helpful to share.
- If you do not have access to the documentation at the time of submitting the form, please continue to submit the form. Documentation can be submitted when you have obtained it.
- We will review all documentation sent to the Student Accessibility Services.
- We may ask for more documentation if needed.
- Once documentation has been approved, the Director of Student Accessibility Services will contact the student via their Frostburg State University email to set up a virtual or in-person appointment to review accommodation requests.
- If you have any questions or concerns, please be sure to ask at this meeting.
- The student MUST request the accommodations before they are put into place for the semester.
- Accommodations are not automatically generated and sent out each semester.
- The student must contact the Student Accessibility Office at the beginning of each semester to request their accommodations.
- Accommodations ARE NOT RETROACTIVE.
